Aged Care Support is able to provide advice, support and assistance to elderly people and their families with the set up and ongoing care in the home or aged care facility. Our services range from companionship through to high level care.
Sometimes the family doesn't know where to start and having the advice of someone who can provide support through the process can make a real difference to the care and experience of your family member.
By having an experienced Registered Nurse guide you through this process we will together be able to provide the best care for your elderly family member. This could include the initiation of district health board funded in home care and subsidised prescriptions if you qualify.
This includes assistance with medical care, care after a hospital stay or illness, ongoing GP and outpatient appointments and assistance and advice with transition from home to residential or hospital care.
A free of charge initial meeting with family members can set out a plan of care for your family member in which we(TM)ll determine the level of care in assistance that will be needed. The care that we provide could be a one off arrangement or an ongoing plan of care where we are heavily involved in all aspects of the elderly persons care.
